Families and Communities Together (FACT) is an interagency initiative designed to assist families of children who are having difficulty in their home, school, or community; who have numerous complex needs; or who are not making adequate progress despite multiple agency involvement. FACT currently targets children in grades K through 8 in eight school districts throughout Wayne County. The program is a direct service provided by the Wayne County Department of Aging and Youth.

The Children & Youth Services Clinic treats youth, ages 3 to 18, for emotional and mental health problems.  Therapists, counselors, nurses, and psychiatrists are available to treat your child in a team approach.  Intake Appointments can be scheduled by calling the above number or by walking in.  Costs are covered by most insurance, but a sliding-scale fee is available.  Treatment is recovery-focused and family centered.  Specialties include the treatment of ADD/ADHD, depression, anxiety, trauma/abuse, and crisis intervention.  Some in-home outreach services are available, based on need.

The Children’s Single Point of Access (C-SPOA) is operated by Wayne County government to enable 

families’ easy, streamlined access to the mental health service system regardless of their financial 

resources or insurance status. While C-SPOA does not provide any direct services, it can help a family to 

access the complete continuum of mental health services for a child. If you are in doubt as to whether 

the child about whom you are concerned should be referred to the C-SPOA, please make the referral.

A Health Home is a care management service model whereby all of an individual's caregivers communicate with one another so that all of their needs are addressed in a comprehensive manner. This is done primarily through a "care manager" who oversees and provides access to all the services an individual needs to assure that they receive everything necessary to stay healthy, out of the emergency room and out of the hospital. Health records are shared among providers so that services are not duplicated or neglected. Health Home services are provided through a network of organizations – providers, health plans and community-based organizations. When all the services are considered collectively they become a virtual "Health Home."  Adult HHUNY services also available.
